Job Description

Summary

The position is under the supervision of the ADAPP Director. The employee is responsible for planning and implementing services related to Mental Health education, consultation, referral, and individual counseling on mental health issues. This is a professional position dealing with the assessment coronation and treatment of mental health issues. The employee develops individual client treatment plans in consultation with the ADAPP Director and other community providers of mental health services, counselors, and departments. The employee is expected to exercise initiative in outreach activities, case management, educational sessions, and individual counseling sessions.

 

Essential Duties and Responsibilities:

  • Plans cooperatively with ADAPP Director and staff counselors, all referrals, and supportive counseling activities.
  • Interviews prospective clients, completes alcohol/drug assessments, mental health evaluations, and develops treatment plans, in coordination with the ADAPP Director, and Community Providers and ADAPP staff counselors.
  • Complies with the confidentiality act requirements by the mental health and chemical dependency; management information system and keeps accurate client records.
  • Carries minimum caseloads of 20 clients per annum and counsels each, as individual treatment plans require.
  • Educates and or counsels families, as needed, on home visits.
  • Presents written/oral reports to the Director every week.
  • Establishes a working relationship with all resource agencies, staff, and the community.
  • Facilitates community awareness of the ADAPP Program by providing mental health educational groups.
  • Attends all staff meetings, workshops, professional seminars, and all other required training sessions
  • Develop personal work skills improvement objectives in consultation with the ADAPP Director.
  • Maintains current CPR/Red Cross First Aid Card.
  • May transport clients to agencies, pertinent to treatment.
  • Complete and assess diagnostic mental health impressions.
  • Ability to establish and maintain an effective working relationship with clients, families, and community providers.
  • Excellent communication skills in writing, speaking, and group interaction.
  • Other duties may be assigned.
